Neighbourhood Care Scheme

For over 30 years, the Neighbourhood Care workers at Time & Talents have been helping adults aged 55 years and above. We know that later life can bring changes and challenges, and sometimes it helps to have someone on your side. Our team is here to listen, offer guidance, and provide practical support, helping you stay connected and feel supported in your community. Common challenges include loneliness, isolation, poor mobility, or limited family/friend networks. Our aim is simply to make sure you have the support that’s right for you, whatever that looks like.

How does our Neighbourhood Care Scheme work?

Think of our Neighbourhood Care workers as friendly professionals who have a good overview of local services in Southwark and how they can be accessed. If they can’t help directly with your challenges, they will be able to point you in the right direction to someone who can.Working across Rotherhithe, Surrey Quays, and Bermondsey, once your referral has been received, a member of our Neighbourhood Care team will contact you to discuss your needs and how we can support you.

They will then make an appointment to see you, either at our office in Rotherhithe or in your home.During this meeting, they’ll take the time to really understand what matters to you and connect you with the support that fits your situation. Our approach is fully person‑centred, focusing on local activities and services that match your interests, needs, and goals.Many people come to us facing challenges like loneliness, isolation, reduced mobility, or having a limited support network, and we’re here to help. We also welcome individuals who are fully independent but looking to build new social connections and enrich their daily lives.

As well as providing guidance and support, the Neighbourhood Care team can also refer you to Time & Talents’ specialist programmes, such as our adult social groups and befriending projects. Through these groups, you can make new friends and have regular connections while remaining independent.They can also signpost you to access other suitable services in Southwark, such as specialist support with housing, health, and social care.Our team welcomes referrals from a variety of sources, including social services, healthcare providers, voluntary organisations, friends, family, and individuals themselves.

Who can access our services?

Anyone living in the orange catchment area below can access our services. Living outside this area? Feel free to get in touch, and we might be able to signpost you to one of our partners working in Southwark.

How do I make a referral to the Neighbourhood Care Scheme?

We accept referrals from social services, NHS colleagues, friends and family, along with self-referrals.

Frequently asked questions about our Neighbourhood Care Scheme

Do you do home visits?

Yes. Our Neighbourhood Care workers can visit you in your home if mobility is an issue.

Is this long-term support?

No. Our Neighbourhood Care workers will likely only visit you once to assess your needs, and then they will signpost you to the relevant assistance, either internally to other Time & Talents services or externally to community partners. If your circumstances change in the future, you can contact us again for further support.

Do you help with form filling?

We can help to fill out forms which give access to transport options such as Dial a Ride or Taxicard. We can not help with filling in Benefits or Housing forms; however, we can signpost you to community partners who do.

Can you help with my debt or housing?

Yes. We can have initial conversations with you about your situation, and then we will signpost you to the relevant community partners for further assistance.

Can you support me if I’m outside the catchment area?

Limited support, such as minibus access for specific groups, may be available. Contact us to discuss further if you are outside of the catchment area shown above on the map.

At what age can I receive support?

You must be 55 or older to access our Neighbourhood Care services.

Meet our Neighbourhood Care workers

Deborah Jordan

I have over 25 years of experience working in the charity sector, supporting people to overcome challenges and empowering them to make their own choices. I’m passionate about helping others have positive experiences — whether that’s through attending our groups and activities, or by seeing someone who was once isolated gain confidence and connect with others.

I enjoy my role as a Neighbourhood Care Coordinator because of the flexibility, variety, and the chance to provide a warm, inclusive, and non-judgemental service that helps our service users achieve the best possible outcomes.

Philomena Ofodu

I’ve spent the past five years working closely with older people in the community, and it’s a role I’m truly passionate about. Supporting their well-being and helping them stay connected is deeply rewarding. Outside of work, I actively campaign and advocate for health awareness to promote better outcomes for all.
